H4 visa Documents
Documents to take for H4 visa
1) Form 156 and 157 and one photo.
2) Appointment letter mentioning the time and date for interview
3) Notice of Action I-797 (Original)
4) H1-Petition Copy of husband
5) Marriage Certificate (Original)
6) 30 (max) Photos of wedding (must include samples of all events)
7) Wedding Invitation
8) Recent (2 months) pay slips of husband
9) Letter from husband's employer which states that he is employed in that company
10) W2 and copies of Tax returns filed in the last couple of years
Not all the above documents might be seen or verified at the time of interview. But, not having them might harm the applicant.
For Marriage Certificate, here are some of the documents required
1) Wedding invitation
2) Driver's License (address proof) of both
3) SSLC (10th) Marks card (name and DOB proof) of both
4) 5-6 copies of double-passport size photo having husband and wife in the picture
5) Three eye witnesses - their full details with signature of them
6) Application form to apply for a marriage certificate completely filled and signed
NOTE: I was told, passport doesn't hold as proof for any of the details when applying for marriage certificate.

I have online copy of statements, do I need to attest them?.

Having recent 2 months of bank statement should be sufficient. Just a proof (print out) is enough. No need to attest.
Are you sure we need ORIGINAL I-797?
I have lost it. but have a copy of it. and planning to file H4 for my spouse.

You need the Original I-797. Please check with your travel consultant about this.
I was looking for H4 related documents and came across ur blog. Does a h4 visa applicant require a sponsor letter from the spouse company. If so what would be the format of the letter.. Hope you can help me in this.

Letter from spouce company is required to prove that you are currently employed and will remain employed in near future.
Format is...
"To Whomsoever it may concern"
...(name) is employed in our company as ...(designation) since ...(date of joining). His conduct has been found good during his employment.
